What we collect and why

This booking service collects your name, date of birth, email, mobile number, selected service, appointment date and time, consultation mode, request status and acknowledgement of this notice and the terms. Date of birth helps identify the correct patient. These details allow Simon to assess and manage your request, check availability and send appointment updates. Your selected service may reveal information about your health. We also retain notification records and technical information used to operate the service.

Providing details is voluntary, but all booking fields are required to submit online. If you prefer another way to book, contact the clinic. Do not submit medical history, documents or urgent concerns through this booking form.

Who receives information

Simon reviews requests in the restricted admin area and checks or updates Dapple manually. This service uses ChatGPT Sites and Cloudflare for hosting and storage, and Resend to send transactional emails. Providers process information needed to deliver those services. We may also disclose information where required or authorised by law. This form does not subscribe you to marketing.

Appointment emails contain timing and location information; they omit your date of birth, phone number and selected treatment. Email is not guaranteed to be confidential and may be accessible to anyone with access to your inbox. Check your email address carefully.

Storage and overseas processing

Booking records and notification records are stored in the hosted database; access to the admin area is restricted to Simon’s authorised account. Providers may process data outside Australia. The countries, provider arrangements and relevant safeguards remain to be verified before this draft is used for public bookings. We do not promise Australian-only storage.

Retention, access and correction

Contact Simon to request access to or correction of your information, or to discuss deletion. Records must be retained where applicable health-record obligations require it; deletion may not always be available. The retention and disposal schedule for booking-only records is being finalised before launch.
